AGREEMENT AND CONSENT

This agreement describes the expectations of the coaching process, and includes the terms and conditions of Heather Wise Health Coaching Services ("the coach") and you ("the client."). Please review and acknowledge your acceptance of these terms.

SERVICES

I will provide you the coaching services we have agreed upon and outlined in my pricing and packages email. Coaching is not therapy and I am not a medical professional. Rather, coaching is a client-centered process designed to help you identify your thought patterns, learn new wellness skills and meet your goals. Our process will include private virtual sessions with discussions and exercises to help you find clarity and devise routines and actions that improve your diet and lifestyle, to develop your desired mindset, and to help you maintain accountability for your success.

You acknowledge that:

1. Heather Wise Coaching services does not offer legal, financial or medical advice and are not a replacement for those or other professional services. You will not use Heather Wise Coaching services in place of these professional services.

2. Coaching is not counseling – Heather Wise is not a licensed health professional and does not diagnose or provide advice or treatment for social, personal, psychological or mental health issues. You understand and agree that you are fully responsible for your well-being during your coaching sessions and subsequently, including your choices and decisions.

3. Coaching is not prescriptive. As a client-driven process, you alone are responsible for evaluating your choices and choosing appropriate actions.

     

SCHEDULING CHANGES AND CANCELLATIONS

Please provide at least 24-hours advance notice to cancel or modify your appointment time. You may use the online appointment scheduler to make changes to your scheduled time. If you cancel less than 24 hours in advance you may be subject to a cancellation charge. 

TERMINATION

Either of us may terminate this agreement, including the services with written notice. If this agreement is terminated, payment is due for services provided up until the effective date of termination. If necessary, I will provide you with a separate invoice for the final amount due within 10 days of the effective date of termination.

CONFIDENTIALITY

Our coaching relationship and the content of every conversation is confidential unless you consent to the release of specific information or I am required to disclose information about our relationship pursuant to a valid court order or subpoena (the coach-client relationship is not subject to any legally recognized privilege). If I believe that you are involved in illegal activities or there is a likely or imminent risk to yourself or others, I may be required to inform appropriate authorities.

DISCLAIMER OF HEALTH CARE RELATED SERVICES

The Coach encourages the Client to continue to visit and to be treated by his/her healthcare professionals, including, without limitation, a physician. The Client understands that the Coach is not acting in the capacity of a doctor, psychologist or other licensed or registered professional. Accordingly, the client understands that the Coach is not providing health care, medical or therapy services,

The Client has chosen to work with the Coach and understands that the information received should not be seen as medical or nursing advice and is certainly not meant to take the place of your seeing licensed health professionals.

LIMITED LIABILITY

You acknowledge and agree that no guarantees or warranties of any kind or nature, whether express or implied, will apply to my services, and you accept sole responsibility for creating and implementing the decisions, choices, actions and results that arise from or relate to my services. In no event will I be liable for any damage or loss that arises from or is related to my services. You hereby release, waive, acquit and forever discharge Heather Wise Coaching, any agents, successors, heirs and employees from every claim, suit action, demand or right to compensation for damages you may claim to have or that you may have arising out of acts or omissions by yourself or by Heather Wise Coaching as a result of the advice given by your coach or otherwise resulting from the coaching relationship contemplated by this agreement.
